What causes one to get extreme penile enlargement as a consequence of priapism?

Why do humans have such massive penises compared to other great apes? Our penises are way bigger in both absolute terms and relative to body mass. What evolutionary pressure lead to this development?

One of the ways the penis stops responding to testosterone (and other androgens) is by downregulation of the androgen receptors in penile tissue. The high doses of various androgens present during puberty subsequently causes the number of androgen receptors in penile tissues to fall. This negative feedback loop is one of the mechanisms that causes the penis to eventually stop growing (there's probably other mechanisms in play too).

Micropenis and other types of underdevelopment of the penis in boys and men is treated with various hormones (hCG, DHT, kisspeptin all are used for this purpose). The female clitoris, which is homologous to the penis and comes from the same embryonic progenitor cells, also respond to these hormones with growth. But the adult, fully grown penis does not respond to the very same hormones with growth. Micropenises and clitorises have not experienced this negative feedback loop of high level of androgens and therefore still respond to them with growth.

If it were possible to increase androgen receptor density in the adult penis, perhaps it would respond to various androgens with tissue growth? It'd be like a second puberty for your penis.

It's funny, I recall reading posts from around 2008-2014 where people hypothesized that you could grow a bigger penis by inhibiting or weakening the crosslinking of collagen and elastin, which is exactly was done in the study.

There was a guy who injected chemicals into his penis with the same goal and mechanism of action in mind. He injected papaverine which causes intense erections and also weakens collagen like in the study. He didn't use a vacuum device but his idea was that the long lasting erections from the papaverine would exert force on the tunica albuginea and buck's fascia of the penis. He did this for several months and documented his gains, he went from 7.5x5in to 9.5x6.5in with picture proof. I can link his posts if anyone wants.

This study seems to confirm such a penis enhancement method works, what a funny coincidence

Suppose that there are 2 penises. One is an adult one and the other is a pubertal one.

The pubertal one responds to various androgenic hormones and grows, but the adult one does not respond to these very same hormones with growth. Why is this? What mechanism is at play here?
